Instructions
- Add chicken, crushed tomatoes, onion, garlic, Italian seasoning, salt, and pepper into the slow cooker.
- Pour in chicken broth and stir to combine.
- Cook on low for 6 hours.
- Stir in heavy cream, mozzarella, and Parmesan cheese until melted and creamy.
- Garnish with fresh basil and serve hot.
